TCS HOCKEY ROSTER/WAIVER FORM
Please read the entire first page and fill out the second page.
Failure to turn in your waiver/roster BEFORE YOUR FIRST GAME will result in NO PLAY and your game will be forfeited .
All players names & coaches must be on the roster prior to your first game along with the player's parent/guardian name & signature.
To make things easier, download the PDF & pre-fill out the form from your computer; print it and then get parent’s signatures at the rink before your first game.
Important notes below:
No Player or Goalie can play on 2 teams in the same AGE group. Ex: 10AA & 10A is NOT allowed. See TCS Rules for more information.
If you have a player missing the first game, please list their name on your roster. Alert our tournament director when checking in & we will assist you with the next steps. Any player who is left off prior to your first game will not be able to participate in the tournament.
All rosters will be locked once your first game has started. You will not be able to add or make changes to your roster.
Please be prepared to show proof of players DOB with a birth certificate (a photo or copy) or passport if needed throughout the tournament.
In addition to your roster/waiver; each team should be prepared to fill out a scoresheet prior to each game, or bring at least 6 roster stickers for the entire weekend.
Emailed rosters will not be accepted.
